All Generations Home Care’s Chronic Disease Management Program provides Clients, Families & AGHC’s Staff with the education and tools needed to help them cope with & manage chronic diseases. The program helps clients manage stress, engage in physical activity & good nutrition, and communicate more effectively with health care providers. This collaborative practice will assist in achieving the goals of Prevention of disease progression, Prevention of Hospitalization and emergency room visits & improve overall Health status.
